Completed Health Questionnaire and payment are required prior to scheduling. Eye photos are requested, but are not required.
1 1/2 to 2 hour appointment includes: 16-week personalized protocol
**Eye Photos may be requested more than once if the quality of the photo is not good
enough for an accurate analysis.
**THE COST OF FORMULAS IS NOT INCLUDED IN THE FEE FOR SERVICES
**You are never required to purchase the suggested formulas listed on your protocol.

Morse’s Health Center Cancellations and Refunds Policy:
If you must cancel your scheduled appointment for any reason, we ask that you do so at least 48 hours prior to your appointment.
We will happily offer you another appointment if you choose to reschedule. If you are requesting a refund and cancellation of your scheduled appointment, we can provide this as a refund to the payment method used to purchase your appointment, or as a credit on file to be applied toward a future service or order from Morse’s Health Center.
Refunds will not be available for no call/no show situations. A credit will be added to your file to be applied toward a future order.
There is no expiration for credits on file.
