Completed Health Questionnaire and payment are required prior to scheduling.
1 1/2 to 2 hour appointment includes: Eye photos and 8-week personalized protocol.
After your consultation you will have 8 weeks of email correspondence with your counselor. The 8-week correspondence window starts 2-weeks after receipt of your protocol.
THE COST OF FORMULAS IS NOT INCLUDED IN THE FEE FOR SERVICES
PLEASE NOTE:
- You do NOT need to purchase our eye photography service separately, as it is already included in your consultation visit at our office.
- You are never required to purchase the suggested formulas listed on your protocol.
- Completed Health Questionnaires, eye photos and any additional information can be
emailed to assistant@morseshealthcenter.com
Morse’s Health Center Cancellations and Refunds Policy:
If you must cancel your scheduled appointment for any reason, we ask that you do so at least 48 hours prior to your appointment.
Appointments cancelled less than 48 hours before the scheduled appointment time will be subject to a $75 late cancellation fee. This fee will be deducted from any eligible refund or, if applicable, charged to the payment method used to book the appointment.
We are happy to help you reschedule your appointment. If you are requesting a cancellation and refund, we can issue your refund to the original payment method used to purchase your appointment or provide a credit on file to be applied toward a future service or order from Morse’s Health Center, less any applicable late cancellation fee.
Refunds will not be available for no-call/no-show appointments. Instead, a credit for the remaining balance, if any, will be placed on your account and may be applied toward a future order or service.
There is no expiration on credits kept on file.
